Position Summary:

A Personal Care Support Assistant is a member of the home care team who works under the Supervisor of Nusing or the Patient Care Coordinator and in accordance with the client's plan of care. This person is responsible for reporting on the client's personal care needs, on change in the client's conditions and on any observed problems that affect the client.

Qualifications, Skills and Competencies Required

  • High School Diploma or GED
  • Completion of a state approved CNA certification training course or ability to pass a two Part PCA competence test
  • Ability to work as part of a team and to handle multiple tasks safely and effectively
  • Good communications skills, including the ability to communicate Sensitive information with empathy ("bedside manner")
  • High tolerance for working under pressure and handling critical situations with a cool head
  • Demonstrates basic technical skills which facilitate an Optimal level of functioning for client, recognizing individual, cultural, and religious diversity.
  • Ability to assist clients with basic personal care skills like bathing, grooming and dressing, toileting etc.
  • Demonstrates the ability to identify Psychosocial needs of all indi processes
  • Demonstrates the ability to identify and implement safety and emergency procedures
  • Demonstrates the ability to use standard and transmission based precautions to prevent the spread of infection
  • Demonstrates behavior which maintains and respects individual rights and choices; and Promotes independence, religion, life-style, sexual preference, disease process, or ability to pay

Duties and Responsibilities

The Personal Care Support Assistant must provide a combination of basic personal care activities or homemaking services, those who, because of age, frailty, medical disability and do not have significant others willing and able to provide care and themselves or function on their own Independently. Each Personal Care Support Assistant will be Provided with a client’s shave the Opportunity to clarify any assigned duties with either the Supervisor of Nursing or Patient Care Coordinator.

Tasks may include, but are not limited to the following (except by law):

Personal Care Tasks

  • Bathing (tub, shower or bed)
  • Routine skin and hair care
  • General mouth/teeth care (including dentures)
  • Assisting with dressing
  • Assisting with toileting

Housekeeping Tasks

  • Vacuuming/Sweeping/Mopping
  • Dusting
  • Emptying Trash
  • Changing Linens and Doing Laundry
  • Cleaning rooms/bathrooms
  • Cleaning stove and refrigerator

Ambulation/Transfers

  • Assisting in/out of bed and/or wheelchair with or without the aid of a lift
  • Assist with walking
  • Encourage Physical Activity
  • Promotes and respects the individual right to make personal choices to accommodate their needs

Proper Nutrition

  • Preparing meals and washing dishes
  • Assisting with eating and drinking fluids

Encouraging Medically Related Activities

  • Observing and reporting changes in client's condition
  • Picking up prescription drugs
  • Accompany client on medical appointments as time permits
  • Reminding client to take medication

Home Management

  •  Grocery Shopping
  •  Assist with bill paying, food stamps and other application processes
  •  Assist with scheduling medical appointments
  •  Other errands as needed
